Schwarzenegger has had heart problems for a very long time. He had a valve replaced something like 25 years ago. The speculation then was he damaged his heart taking anabolic steroids.
He has undergone three heart surgeries in the past, including open heart surgery in 1997. The valves each lasted 21 years. The actor had replacement valve surgery in 2018, with doctors replacing one of two valves, and he went on to have the other replaced in 2020.

The human heart is simply not capable of long term sustained pumping of blood thru a massively enlarged muscular body like that. Could also be from prolonged steroid use, too. He had to keep his physique somehow with the filming schedules he was keeping during the 80's thru the early 2000's.
