He sees patients and he does clinical research. He will email you back too. I personally lost 200 pounds with it. It’s a life changer. The one statement I didn't see in the OP is - 'they' are looking for a change in their plans. If 'they' aren't doing this for themselves, they will never follow through.

You haven't mentioned their financial status but if they are 'comfortable' there are several 'prepared meal' companies that do Keto meals and prepackage and send them to the front door of your friends. This way they could have their variety in meals and control their intake if they would just agree to follow the plans and not cheat on the side with high carb or processed foods. If their knees are bad, they would probably appreciate not having dishes and pans to deal with - just eat out of the individual containers and throw away. If they agree to comply for 30 days, the benefits they see will convince them to continue on for longer. If they are serious after that, either they could enlist the DIL to prepare similar meals or else learn the rules themselves.
Animal fat is good for brain health. Stay away from seed oils such as Canola, Soy, Corn, Safflower, Sunflower etc etc. Use Ghee to cook or Olive oil and no other.
Grass fed beef and fresh organic vegetables only. Nothing processed at all.
No sugars especially.
I would add virgin coconut oil to your oils. And take a look at intermittent fasting which really gives your body a rest and time to reset and recover.
Take out the olive oil, too. Use PURE coconut oil instead. If it has a stronger scent, it isn’t pure. It should set at 72 degrees F. Believe it or not, Olive Oil is almost as bad as other seed oils. I know, I was surprised, too, when I learned about it.
Tallow and meat fats are the best, as our bodies are meant to absorb them.

Peanut butter, and tomatoes are also inflammatory I still eat tomatoes but only with tacos. I use a few taco chips and make a salad. Some things you just can't compromise on lol but I still try to keep it under 20 carbs a day, that's strict keto for me though.

Yes, as you noted, vegetable oils are harmful to practically everyone. Coconut is good.

This is probably the best advice. Cut carbs to zero if possible. Eliminate: Cereals, Alcohol, Fruit Juices, Sodas. You're gonna get a few grams of carbs no matter what you do, no big deal, just avoid them. Also, 20-30 minutes of exercise daily. With bad knees, consider swimming or maybe kayaking. Is there a city aquatic center nearby?
Low carb living is the way for me. Mostly because I'm diabetic and it helps jerri control my blood sugar and weight.
First rule of low carb... shop only the outside aisles of the grocery store. For the most part, that where the fresh foods are. Stay out of the aisles where all the pre-packaged, overprocessed crap is. Avoid white foods except cauliflower - rice, potatoes, pasta, breads. Sadly, they may have to actually cook.
My best advice? Find an actual nutritionist in your area who may be able to customise a diet to address his specific issues. They may be able to pinpoint what he eats and it's affects that may be causing some of his issues.

Man, thats tough. Would depend on their goal. Where are they, where do they want to be, and when. Then they will need to see a nutritionist.
First start is always exercise. And keep at it till they dont swell with water after doing so. Once they make that part of their life their body will push them in the direction of what their body needs to maintain it.
If they won't do that, Id suggest, and this is just my opinon, cut meat out 3 days a week. No dairy, no sugar, nothing canned, no breads at all, no pasta, limited salt.
This is what I try to do: No processed foods, very little carbs/sugar and only from natural sources. No snacking between meals, 2 meals a day. Have a late breakfast - several eggs (with yolk) and dark leafy greens, get eggs from a farm directly. Dinner - a quality protien and vegetables, soups.
I see you've gotten some really good advice already on the dieting part. Have you also considered mental health? The mind affects the body in a multitude of ways. Not only has this been known in medicine for ages (think of traditional Chinese medicine) but it has been scientifically confirmed with the healing effects placebos can have. A negative issue may be holding them back, they may feel like they are under a lot of pressure... Whatever it is, maybe there's something there as well that will help them get better. My back didn't get any better until I went to a hypnotist who figured out some deep-seated trauma. I couldn't walk properly for a while and mentally I felt like I was stuck. After the session with the hypnotist, my pain was slowly fading away and I'm much better now. Hope your friend gets better!
I'd set up an appointment with a very cool MAGA nutritionist with credentials. Changes to diet are life changes, so it is best to do one thing at a time. That's the whole idea behind Lent - give up one thing, substitute another, then it is a lifestyle. Rather than you trying to be the nutritionist for them that implements a new diet and exercise plan, the nutritionist really knows the tricks of the trade to listen and suggest. You sound like a great friend for caring.
Did they get a knee surgery already, and their post-op? Can I recommend supplements like vitamins, ashwaganda which controls pain and inflammation, bromelein, a pineapple enzyme that controls pain and inflammation particularly well with joint pain, and quercetin, same again.
I personally take the above with willow bark, also a pain killer, and I feel like I did in college. I also had foot and knee surg. for prior injury related ortho issues.
Let us know how it goes, please. For me I went from nearly ready to apply for disability to fitter than I've ever been because the inflammation and pain finally has a strong antidote that is not an opioid of some kind. I also quit ETOH, white sugar/flour and am mostly keto, but that took time for me to get there.
Carnivore/Keto/Paleo. Adamantly no sugar, no pasta no bread. She will be less hungry, too, the longer she is on a mostly meat diet. It is all we do at my house. Had to! Eating crap was breaking us down fast.
I went from being really fat to watching it melt away. I do 2 things. First and foremost is walk every day. Get them to walk as far as possible every day. Second get the biotrust ageless multi collagen which is absolutely amazing. I have watched it melt the fat off for the last 2 months I went from super fat to very normal within a short period of time. But the walking is super important. I do not diet I eat what I want. You can get these multi collagen products at drug stores but they are not the real products use the bio trust they are the best.
Look add a box of raisins. Look at the ingredients. Raisins. That's it.

A nine minute walk a day, can make all the difference. If the knee does not allow walking, a stationary or table top bicycle will do! It keeps the knee greased! Just pedal slowly around.
My friend had knee surgery and got a tabletop bicycle you can put on the floor while you're watching TV and just pedal!

Vitamin D is EXCELLENT, and very, very necessary, so good points. With the vitamin D (try chaga), make sure that it is balanced with some good natural sources of Vit K (like dark poultry meat, free range medium eggs, sauerkraut, etc. The Vit K helps the body send the Calcium (natural by-product of Vit D absorption) into the bones, instead of the bloodstream (Ca2’s primary purpose is blood clotting after a wound’s signalling of the histamine response, so any EXCESS needs to go into the bones). However, since Calcium inhibits Iron absorption, space out the food intake so that Vit D/Calcium/etc concentrates are eaten a few hours apart from the iron-rich foods.
Also, there are classes under the umbrella GLA:D that are exercises geared towards those trying to avoid knee and/or hip replacements. There are different levels to accommodate each person. See if there are any in your area. Athletic taping may greatly assist your friend(s) before each class so as to allow the exercises to be performed with significant pain reduction. Taping is temporary, though.
I think one of the best additions I have made supplement wise is the addion of vitamin D and K. I take 10,000 IU per day and a Super K from Life Extension. I improved from about 25 to 65 and it really has helped so many areas of my health...especially arthritis and some weight loss. D is involved in a multitude of processes in the body and the K puts calcium in the bones instead of the arteries. I believe God gave us free sunshine to heal our bodies but due to climate, season or malabsorption I really believe most of us need a boost.
I would personally recommend ALL of the work of Ray Peat PhD. He has many articles fully referenced at the bottom. He sells nothing, but puts his research up for FREE because the bulk of it was paid for by taxpayers anyways.
He explains physiology through a bio-energetic lens. As long as your friend is breathing he can heal, NO vegetables necessary.
In my opinion your friend has very high levels of inflammation. The strategy is to get that under control. Btw, excess Iron is highly inflammatory and feeds bacteria, donate blood if iron levels are too high. PUFA is highly toxic and inflammatory, so no more Mongolian grill. Cook at home with stable saturated fats like coconut oil and butter.

Diets don't work.... drink a lot of water before eating, eat your veggies first and enough til you tire of them. Cap off the meal with whatever else you want. Veggies should be as many colors as possible and 2/3rd of what you eat. Fruit of different colors as well, but not too much because of sugars. Lastly eat until not hungry, do not eat until full.
Way back, Richard Simmons taught to pay attention when you're eating to when you put down your fork. Most people will eat a good portion and then get a natural pause in their eating and that's when you should probably stop because you've actually had enough.
Not exactly what you asked but they need a mission or goal that facilitates the idea of being healthy and if they are unhealthy now it's unlikely they have one.
Finding one depends on the person but it should transcend the person, so simply obtaining health or for finding a spouse isn't good enough. If they are a Christian this becomes alot easier frankly.
Non gmo foods. No more fast food and drink only water or milk for at least a week. Watch and see how better your friend feels and no longer craves the sugar and carbs. 😉
One thing I forgot to mention: SLEEP. All the people I know who started keeping late hours got fat. Normal sleep regime is essential for good health; an old friend used to say an hour before midnight is worth 2 after.
Start with Ivermectin to eliminate a broad array of parasites. Once a month is needed for elimination and prevention. A daily capsule of 500mg of Quinine as a herbal supplement. It is what HCQ is a synthetic version of. It can be easily ordered through the mail. It is an anti-inflammatory, as well as an anti-malarial. Malaria isn't the issue, the inflammation is. A balanced diet is best with modest sized portions. Drink plenty of water, especially with meals. This will fill the stomach and enhance the feeling of a satiated hunger. Avoid all artificial sweeteners and processed sugars, these are inflammatory. This is the hardest one, do research on the benefits of fasting, work up his courage to do a 7 day fast with only water. This is very difficult for the first 3 days, but after that it gets much easier. Do this once every three months. This will allow the body to cleanse and the digestive system to rest. Antibiotics wipe out good bacteria as well as bad, so get probiotics through either reintroduced after pasteurization or unpasteurized yogurt or fermented foods, Korean Kimchee or Sauerkraut, possibly kambucha. Doctors aren't supposed to do joint surgery on really obese people. The joints just blow out from the weight.
My brother died in his 50s from a massive stroke or heart attack. They weren't sure which because his body was too large for the CAT scanner at the local hospital. He was obese his entire life and actually didn't eat huge amounts of food. He had one successful diet and lost a lot of weight in his 20s but then put it all back on plus more. Just weeks before he died, a health scare finally got him to get serious about his weight and he'd actually lost some. But it was too late.
My point is that, like alcoholics, some morbidly obese people just don't get it or consider what morbidly means. They might not listen to you unless they're staring death in the face and perhaps not then.
I'm not trying to discourage you from trying, but don't feel bad if they don't listen.
BTW, we just lost another very obese family member. She'd had multiple joint replacements. Each time her mobility got worse until she couldn't walk without a walker and then, the past few months, couldn't walk at all. We begged her to watch her diet and to exercise. She really did have horrible eating habits and was like a junky with junk food.
Keto, but even though I fail often, I'm still dropping weight very gradually, and all my numbers are gradually moving the right way.
Some of the best foods are keto: hamburger, bacon, cheese, lettuce, tomato, everything but the bun. And the keto foods are satisfying. I love scrambled eggs with some cheese and a slice of ham. It's quick and really holds me until lunch time.
I saw a nutrionist several years ago. She didn't want any foods or drink eliminated. I was given a recipe for a vegetable soup that was cabbage based and built on with other veggies from there. She told me it was limitless throughout the day as far as consumption. It really regulated my system and alleviated cravings. It changed my relationship with food. I start feeling full and icky, I whip up a batch.
First I would do a simple test to see how well he tolerates carbs, many diets fail because people don't know how much amylase they produce in their saliva. Simple test: get a plain unsweetened biscuit (in UK we call them water biscuits / cream crackers / matzos) and see how quickly you can taste the sweetness: before 18seconds you are good; 15-28s - not so good; 28s + not good, cut down on carbs, have a high protein diet.
Avoid "low fat" and processed food. Eat as much fresh fruit, salads and vegetables as possible - raw carrots are great for curbing appetite, tomato juice is very filling with few calories. Cut out alcohol, sugar and snacks. Eggs are good, I have a boiled one for breakfast most days. Try to exercise if possible - get MAGNESIUM oil (simple to make, just melt Mg crystals) for stiff joints. Best with DMSO - if you can get it, helps absorption; just be sure to read up on how to use it (30% strength).
Do not cut out fats, they are important for brain, heart health and vitamin absorption. Good vitamin and mineral supplements help, Tyrosine too. Also involved in a huge number of processes in the body - worth reading up. So far it keeping severe arthritis and my carpal tunnel at bay. Recently sprained my ankle, used a couple of drops with DMSO and was pain free, able to walk and the swelling went down in a couple of days. Wonderful stuff, add a little to my home made toothpaste too, good for gums and teeth. Great addition to a bath. Use daily!
I've been in your situation... I'm an older senior but am in better shape than people half my age, hit the gym 5 days a week, lift significant weights, row, and do strenuous lift routines, eat right, don't smoke or drink alcohol, and am fortunate enough to have great discipline and good genes.
My contemporaries ask how they can get in my shape and I just don't have the heart to tell them that it's probably too late for them, they should have started years ago...DECADES ago. For them to start now would take discipline they don't have, eat foods they won't like, and make it part of their lifestyle.
So unless your friend has tons of discipline, your good intentions might be wasted. Sorry to tell you this.
I have! I have done two over the past few years and retained most of the eating habits since. I lost 30 lbs (but i wasn’t big to begin with) more than that I lost all systemic inflammation, skin issues cleared right up, digestion is 💯 and so much more energy. I recommend it to everyone but people are gonna do what they’re gonna do. I thought at first that it was totally crazy to cut wheat but it’s been the best thing I’ve ever done. Please, please, please watch just five minutes of this vid and you will want to watch the entire vid. This doctor has a lot of insight on what makes people fat and how to get the weight off. Please give this vid at least five minutes of your time.
This person doesn't cook, but has a daughter in law who would cook for them if asked.
You know what this means, right? It means they eat a lot of junk food, which would explain their many health problems.
They might not want to admit it to you, but you can be sure if they do not cook, then they are not getting quality food (unless they are a rare raw food eater, which they clearly are not).
If the person refuses to change eating habits, then nothing else will help. It will continue to be more and more health problems, until the end.

I realize that is a bridge too far for most people, but he had an interesting story he told that could apply to your friend's situation.
He said he got a call on a Thursday or Friday from a doctor friend. The doctor said she had a patient that had been bed-ridden for two years (I think), and was now on her death bed. The medical industry had thrown everything at her that they could think of, and she was likely to die within a matter of days or weeks, at most.
He joked that it was kind of late in the game to call him, but he thought of one thing that might help. He said to go buy several dozen eggs, and have the woman eat as many raw eggs as she could, throughout the day, and see if that helps.
He said he then got a phone call the following Monday. The voice on the other end of the line said, "OK, I am out of bed, now what do I do?" It was the bed-ridden lady. She had eaten about 5 dozen raw eggs over the weekend, and was now out of bed, up and moving around, for the first time in months or years.
Sounds too good to be true, I know, but if you consider what is in quality raw eggs, there could be something to it.
It is the single best source of protein for humans. It has high quality fats, which your friend is likely not getting much of. It has high quality micronutrients, and no anti-nutrients that I know of. They are easy to crack and eat. No cooking. No dishes. No cleaning.
They do not taste like chocolate cake or Mongolian beef, but for a person in dire health, it might be worth a shot.
I eat raw eggs almost every day. Took quite awhile to get used to the taste, and now it is no big deal. I have eaten more than 2 dozen in a day, and often eat a dozen a day.
If you try it, make sure to get the best quality you can find. They must be "free range." No other packaging matters. Cage free means nothing. Pastured means nothing. Pasturized is bad, not good. Vegetarian feed is bad, not good, since chickens naturally eat a lot of bugs. If you can get fertilized eggs from a local farmer, that would be ideal, but you will not find them in any retail stores.
Try a raw eggs only diet for one week and see if there is any benefit.
^ Also, try to cut out all supplements and medications, if possible. They could be contributing to problems that might be solved when the diet improves.
For example, if the diet is causing high blood pressure, they likely take blood pressure meds. If they stop eating a diet that causes high blood pressure, but stay on meds that artifically lower blood pressure, then that is also a dangerous situation.

Commercial peanut butter usually has soybean oil. You need natural or organic for it to be healthy.

This is what I try to do: No processed foods, very little carbs/sugar and only from natural sources. No snacking between meals, 2 meals a day. Have a late breakfast - several eggs (with yolk) and dark leafy greens, get eggs from a farm directly. Dinner - a quality protien and vegetables, soups.
In small quantities.
Agree and source is important. Get a raw organic honey, preferably from the source directly.
I see you've gotten some really good advice already on the dieting part. Have you also considered mental health? The mind affects the body in a multitude of ways. Not only has this been known in medicine for ages (think of traditional Chinese medicine) but it has been scientifically confirmed with the healing effects placebos can have. A negative issue may be holding them back, they may feel like they are under a lot of pressure... Whatever it is, maybe there's something there as well that will help them get better. My back didn't get any better until I went to a hypnotist who figured out some deep-seated trauma. I couldn't walk properly for a while and mentally I felt like I was stuck. After the session with the hypnotist, my pain was slowly fading away and I'm much better now. Hope your friend gets better!
I'd set up an appointment with a very cool MAGA nutritionist with credentials. Changes to diet are life changes, so it is best to do one thing at a time. That's the whole idea behind Lent - give up one thing, substitute another, then it is a lifestyle. Rather than you trying to be the nutritionist for them that implements a new diet and exercise plan, the nutritionist really knows the tricks of the trade to listen and suggest. You sound like a great friend for caring.
Did they get a knee surgery already, and their post-op? Can I recommend supplements like vitamins, ashwaganda which controls pain and inflammation, bromelein, a pineapple enzyme that controls pain and inflammation particularly well with joint pain, and quercetin, same again.
I personally take the above with willow bark, also a pain killer, and I feel like I did in college. I also had foot and knee surg. for prior injury related ortho issues.
Let us know how it goes, please. For me I went from nearly ready to apply for disability to fitter than I've ever been because the inflammation and pain finally has a strong antidote that is not an opioid of some kind. I also quit ETOH, white sugar/flour and am mostly keto, but that took time for me to get there.
Ethanol or ethyl alcohol. Beer, wine, and liquor. Medical types use that abbreviation.
Carnivore/Keto/Paleo. Adamantly no sugar, no pasta no bread. She will be less hungry, too, the longer she is on a mostly meat diet. It is all we do at my house. Had to! Eating crap was breaking us down fast.
I went from being really fat to watching it melt away. I do 2 things. First and foremost is walk every day. Get them to walk as far as possible every day. Second get the biotrust ageless multi collagen which is absolutely amazing. I have watched it melt the fat off for the last 2 months I went from super fat to very normal within a short period of time. But the walking is super important. I do not diet I eat what I want. You can get these multi collagen products at drug stores but they are not the real products use the bio trust they are the best.
Intermittent fasting no diet to start. Overnight, water on 16 hours.
After 2 months mod diet, probably will happen a bit as body recovers.
Quit booze.
Stick to WHOLE FOODS.
Look add a box of raisins. Look at the ingredients. Raisins. That's it.
Stick to whole foods and try to eat organic. Especially processed foods like peanut butter, crackers, cereal. OATMEAL for sure.
Lean meats, not much sugar, either.
VITAMINS!! D3 for sure, 5000ui. Zink and C added to a good multi vitamin.
They're showing a need for antibiotics at times they probably shouldn't, so the extra vitamins are to boost the immune health.
A nine minute walk a day, can make all the difference. If the knee does not allow walking, a stationary or table top bicycle will do! It keeps the knee greased! Just pedal slowly around. My friend had knee surgery and got a tabletop bicycle you can put on the floor while you're watching TV and just pedal! $150 online!
Vitamin D is EXCELLENT, and very, very necessary, so good points. With the vitamin D (try chaga), make sure that it is balanced with some good natural sources of Vit K (like dark poultry meat, free range medium eggs, sauerkraut, etc. The Vit K helps the body send the Calcium (natural by-product of Vit D absorption) into the bones, instead of the bloodstream (Ca2’s primary purpose is blood clotting after a wound’s signalling of the histamine response, so any EXCESS needs to go into the bones). However, since Calcium inhibits Iron absorption, space out the food intake so that Vit D/Calcium/etc concentrates are eaten a few hours apart from the iron-rich foods.
Also, there are classes under the umbrella GLA:D that are exercises geared towards those trying to avoid knee and/or hip replacements. There are different levels to accommodate each person. See if there are any in your area. Athletic taping may greatly assist your friend(s) before each class so as to allow the exercises to be performed with significant pain reduction. Taping is temporary, though.
These points will assist with lifestyle changes.

I think one of the best additions I have made supplement wise is the addion of vitamin D and K. I take 10,000 IU per day and a Super K from Life Extension. I improved from about 25 to 65 and it really has helped so many areas of my health...especially arthritis and some weight loss. D is involved in a multitude of processes in the body and the K puts calcium in the bones instead of the arteries. I believe God gave us free sunshine to heal our bodies but due to climate, season or malabsorption I really believe most of us need a boost.
I would personally recommend ALL of the work of Ray Peat PhD. He has many articles fully referenced at the bottom. He sells nothing, but puts his research up for FREE because the bulk of it was paid for by taxpayers anyways.
He explains physiology through a bio-energetic lens. As long as your friend is breathing he can heal, NO vegetables necessary.
In my opinion your friend has very high levels of inflammation. The strategy is to get that under control. Btw, excess Iron is highly inflammatory and feeds bacteria, donate blood if iron levels are too high. PUFA is highly toxic and inflammatory, so no more Mongolian grill. Cook at home with stable saturated fats like coconut oil and butter.

Diets don't work.... drink a lot of water before eating, eat your veggies first and enough til you tire of them. Cap off the meal with whatever else you want. Veggies should be as many colors as possible and 2/3rd of what you eat. Fruit of different colors as well, but not too much because of sugars. Lastly eat until not hungry, do not eat until full.
Way back, Richard Simmons taught to pay attention when you're eating to when you put down your fork. Most people will eat a good portion and then get a natural pause in their eating and that's when you should probably stop because you've actually had enough.
Smaller plates are actually helpful.

Not exactly what you asked but they need a mission or goal that facilitates the idea of being healthy and if they are unhealthy now it's unlikely they have one.
Finding one depends on the person but it should transcend the person, so simply obtaining health or for finding a spouse isn't good enough. If they are a Christian this becomes alot easier frankly.
Non gmo foods. No more fast food and drink only water or milk for at least a week. Watch and see how better your friend feels and no longer craves the sugar and carbs. 😉
One thing I forgot to mention: SLEEP. All the people I know who started keeping late hours got fat. Normal sleep regime is essential for good health; an old friend used to say an hour before midnight is worth 2 after.
Start with Ivermectin to eliminate a broad array of parasites. Once a month is needed for elimination and prevention. A daily capsule of 500mg of Quinine as a herbal supplement. It is what HCQ is a synthetic version of. It can be easily ordered through the mail. It is an anti-inflammatory, as well as an anti-malarial. Malaria isn't the issue, the inflammation is. A balanced diet is best with modest sized portions. Drink plenty of water, especially with meals. This will fill the stomach and enhance the feeling of a satiated hunger. Avoid all artificial sweeteners and processed sugars, these are inflammatory. This is the hardest one, do research on the benefits of fasting, work up his courage to do a 7 day fast with only water. This is very difficult for the first 3 days, but after that it gets much easier. Do this once every three months. This will allow the body to cleanse and the digestive system to rest. Antibiotics wipe out good bacteria as well as bad, so get probiotics through either reintroduced after pasteurization or unpasteurized yogurt or fermented foods, Korean Kimchee or Sauerkraut, possibly kambucha. Supplement with foods with high vitamin and minerals or capsule supplements for a high quality multi vitamin, vitamin D3, Omega-3's, B complex, zinc (with Quinine it becomes anti-viral), and essential amino acids L-arginine, L-lysine etc. The weight should start to come off gradually, especially fat if he/she concentrates on a keto diet that eschews carbs and sugars. The body cells switch to protected mode and burn 96% fat and 4% protein. The diet should concentrate on these types of foods (in moderation). Joints should start to heal and mobility will help increase fat burn. Low impact exercise is key, no running, jogging or impact type exercise until joints are completely healed. Walking is perfect. To increase endurance and obtain full body exercise, a rowing machine fits the bill very well.

Doctors aren't supposed to do joint surgery on really obese people. The joints just blow out from the weight.
My brother died in his 50s from a massive stroke or heart attack. They weren't sure which because his body was too large for the CAT scanner at the local hospital. He was obese his entire life and actually didn't eat huge amounts of food. He had one successful diet and lost a lot of weight in his 20s but then put it all back on plus more. Just weeks before he died, a health scare finally got him to get serious about his weight and he'd actually lost some. But it was too late.
My point is that, like alcoholics, some morbidly obese people just don't get it or consider what morbidly means. They might not listen to you unless they're staring death in the face and perhaps not then.
I'm not trying to discourage you from trying, but don't feel bad if they don't listen.
BTW, we just lost another very obese family member. She'd had multiple joint replacements. Each time her mobility got worse until she couldn't walk without a walker and then, the past few months, couldn't walk at all. We begged her to watch her diet and to exercise. She really did have horrible eating habits and was like a junky with junk food.

Keto, but even though I fail often, I'm still dropping weight very gradually, and all my numbers are gradually moving the right way.
Some of the best foods are keto: hamburger, bacon, cheese, lettuce, tomato, everything but the bun. And the keto foods are satisfying. I love scrambled eggs with some cheese and a slice of ham. It's quick and really holds me until lunch time.
Keto can actually eliminate diabetes.
I saw a nutrionist several years ago. She didn't want any foods or drink eliminated. I was given a recipe for a vegetable soup that was cabbage based and built on with other veggies from there. She told me it was limitless throughout the day as far as consumption. It really regulated my system and alleviated cravings. It changed my relationship with food. I start feeling full and icky, I whip up a batch.
It's very possible. Every region probably has some miracle green they swear by. Maybe kimchi is their cabbage.
First I would do a simple test to see how well he tolerates carbs, many diets fail because people don't know how much amylase they produce in their saliva. Simple test: get a plain unsweetened biscuit (in UK we call them water biscuits / cream crackers / matzos) and see how quickly you can taste the sweetness: before 18seconds you are good; 15-28s - not so good; 28s + not good, cut down on carbs, have a high protein diet.
Avoid "low fat" and processed food. Eat as much fresh fruit, salads and vegetables as possible - raw carrots are great for curbing appetite, tomato juice is very filling with few calories. Cut out alcohol, sugar and snacks. Eggs are good, I have a boiled one for breakfast most days. Try to exercise if possible - get MAGNESIUM oil (simple to make, just melt Mg crystals) for stiff joints. Best with DMSO - if you can get it, helps absorption; just be sure to read up on how to use it (30% strength).
Do not cut out fats, they are important for brain, heart health and vitamin absorption. Good vitamin and mineral supplements help, Tyrosine too. Best way to start is with what I call a Kaboom drink to get the gut cleansed: liquidise apple, orange + orange juice, banana add Bentonite clay if possible and take charcoal tabs or capsules.

I get Magnesium Chloride Hexahydrate Flakes from The Zechstein Source: all you have to do is melt it - don't buy ready made spray as its overpriced and the crystals can reform if cold. Avoid broken skin or it will sting, even a small scratch; no big deal, just rinse off. Known as the happy mineral as it lifts mood and aids sleep. Also involved in a huge number of processes in the body - worth reading up. So far it keeping severe arthritis and my carpal tunnel at bay. Recently sprained my ankle, used a couple of drops with DMSO and was pain free, able to walk and the swelling went down in a couple of days. Wonderful stuff, add a little to my home made toothpaste too, good for gums and teeth. Great addition to a bath. Use daily!
Whole30 is a great no nonsense way to eat. The Whole 30 book “It Starts with food” is a good read. A lot of good science and common sense.
I've been in your situation... I'm an older senior but am in better shape than people half my age, hit the gym 5 days a week, lift significant weights, row, and do strenuous lift routines, eat right, don't smoke or drink alcohol, and am fortunate enough to have great discipline and good genes.
My contemporaries ask how they can get in my shape and I just don't have the heart to tell them that it's probably too late for them, they should have started years ago...DECADES ago. For them to start now would take discipline they don't have, eat foods they won't like, and make it part of their lifestyle.
So unless your friend has tons of discipline, your good intentions might be wasted. Sorry to tell you this.

I have! I have done two over the past few years and retained most of the eating habits since. I lost 30 lbs (but i wasn’t big to begin with) more than that I lost all systemic inflammation, skin issues cleared right up, digestion is 💯 and so much more energy. I recommend it to everyone but people are gonna do what they’re gonna do. I thought at first that it was totally crazy to cut wheat but it’s been the best thing I’ve ever done. And I’m literally a professional sourdough baker lol.

You know what this means, right? It means they eat a lot of junk food, which would explain their many health problems.
They might not want to admit it to you, but you can be sure if they do not cook, then they are not getting quality food (unless they are a rare raw food eater, which they clearly are not).
If the person refuses to change eating habits, then nothing else will help. It will continue to be more and more health problems, until the end.
In your heart of hearts, you know that is true, even though you wish it were not.
Now that I think about it some more, there is one thing that might we worth a try.
No cooking, no dishes, no clean up to speak of, and can be done by even a bed-ridden person.
I don't know if it works or to what extent it does, but Aajonus Vonderplanitz claimed it did. He was a very interesting man, promoting a raw meat diet.
I realize that is a bridge too far for most people, but he had an interesting story he told that could apply to your friend's situation.
He said he got a call on a Thursday or Friday from a doctor friend. The doctor said she had a patient that had been bed-ridden for two years (I think), and was now on her death bed. The medical industry had thrown everything at her that they could think of, and she was likely to die within a matter of days or weeks, at most.
He joked that it was kind of late in the game to call him, but he thought of one thing that might help. He said to go buy several dozen eggs, and have the woman eat as many raw eggs as she could, throughout the day, and see if that helps.
He said he then got a phone call the following Monday. The voice on the other end of the line said, "OK, I am out of bed, now what do I do?" It was the bed-ridden lady. She had eaten about 5 dozen raw eggs over the weekend, and was now out of bed, up and moving around, for the first time in months or years.
Sounds too good to be true, I know, but if you consider what is in quality raw eggs, there could be something to it.
It is the single best source of protein for humans. It has high quality fats, which your friend is likely not getting much of. It has high quality micronutrients, and no anti-nutrients that I know of. They are easy to crack and eat. No cooking. No dishes. No cleaning.
They do not taste like chocolate cake or Mongolian beef, but for a person in dire health, it might be worth a shot.
I eat raw eggs almost every day. Took quite awhile to get used to the taste, and now it is no big deal. I have eaten more than 2 dozen in a day, and often eat a dozen a day.
If you try it, make sure to get the best quality you can find. They must be "free range." No other packaging matters. Cage free means nothing. Pastured means nothing. Pasturized is bad, not good. Vegetarian feed is bad, not good, since chickens naturally eat a lot of bugs. If you can get fertilized eggs from a local farmer, that would be ideal, but you will not find them in any retail stores.
Try a raw eggs only diet for one week and see if there is any benefit.
^ Also, try to cut out all supplements and medications, if possible. They could be contributing to problems that might be solved when the diet improves.
For example, if the diet is causing high blood pressure, they likely take blood pressure meds. If they stop eating a diet that causes high blood pressure, but stay on meds that artifically lower blood pressure, then that is also a dangerous situation.
